Immunizations
  
Childhood and adolescent immunizations up to age 18 include: diphtheria, tetanus, pertussis, polio, haemophilus influenzae Type b (Hib), measles, mumps, rubella, varicella (chicken pox), hepatitis A, hepatitis B, influenza (flu shot), pneumococcal disease, meningococcal disease and human papillomavirus (HPV).
 
Adult immunizations* include: diphtheria, tetanus, pertussis, measles, mumps, rubella, varicella (chicken pox), influenza (flu shot), pneumococcal disease, meningococcal disease.
*Note: age, history of chronic illness, and time of year will determine the necessity for and/or eligibility requirements for some of these immunizations (for example, the influenza vaccine)
